Same document saving as duplicate

We are having some problems with some specific customers. When checking in a document, it appears to save it again instead of just checking in the changes.

It's as if it doesn't exist inside the vault.

And sometimes this document apparently self-deletes itself within the vault.

What could this be? Is there any way to avoid it?
