Questionnaire in M-Files

Question

Our client asked us if we could create a simple questionnaire in M-iles, as shown in the image. It would be easy to create an object containing all the questions, but the questions are long and the property names are truncated. Can you think of a solution? It's like filling out a questionnaire.

I've seen M-Files being used that way. If you want a UX which is perfect for this then you'd probably need to consider some custom UIX, or external application that pushes data into M-Files via the APIs. 
 That's probably a significant amount of development investment depending upon your skillset, though, so it would be worth trying to weigh up the impact of such a development vs the work needed.
