How to ensure that a person has checked out a file and signed and checked in in a workflow

I have a work flow where a document assigns to user for signature. once they sign they have to click Signed & Approved state in workflow. user is able to click Signed & Approved state without signing the document , how Can I make Signature or Checkout /Check in mandatory
